Sheridan Health Services

Sheridan Health Services is a 501(c)(3) federally-qualified nurse-managed clinic that began in 1995 with providers from the University of Colorado, College of Nursing. We offer a variety of services to encompass all of a patient's needs and strive to provide complete care to individuals with a focus on wellness and disease prevention. Our Youth Health Clinic is a school-based health center (SBHC) which provides primary care services to students for a comprehensive range of physical, mental, and oral health services. SBHCs led to improved educational outcomes and health outcomes, and decrease school absences and emergency room visits.

Appointments

An appointment is recommended. However, if you are ill and need to be seen immediately, please call or walk-in and we will make every effort to see you the same day. If you are 15 minutes late, you will likely need to reschedule your appointment; we apologize for the inconvenience.

Prices for Common Services

Colorado Law requires us to post the full-fee price for the 15 most common services we provide.

The prices listed are an estimate of what you might pay if:

  • You do not have insurance (including Medicaid and Medicare)
  • The service isn’t covered by your insurance
  • You are not enrolled in Sheridan Health Service’s Sliding Fee Discount Program

General Payment Information

Our Enrollment Specialists offer free financial screening for our patients to help determine eligibility for Medicaid, CHP+, commercial insurance, or the Sheridan Health Services Sliding Fee Discount Program.

We accept Medicaid, CHP+, and some private insurance plans.  Currently, we are contracted with Cigna, Anthem, United Healthcare, Rocky Mountain Health Plan and Multi-Plan.  If you are uninsured, you may be eligible for our Sliding Fee Discount Program. You must set an appointment with our outreach and enrollment specialists in order to be screened for eligibility. We are also able to register eligible clients for Medicaid or with the Connect for Health Colorado insurance exchange.??
