FAQ 1:My child is sick…should they stay home from school? Check the How Sick is Too Sick guidelines from the Colorado Department of Public Health and Environment (CDPHE). Also available in other languages – click either link below to find other languages available.

  FAQ 2: My child needs medication at school...what forms are needed? 

For school staff to administer any medication at school a signed order from the medical provider is needed. This order must also be signed by the parent and must be reviewed and signed by the school nurse consultant before the medication can be given at school. The 3 most commonly used forms are available below:

 

  1. Medication Administration Permission Form: Use this form for general medications (such as antibiotics, ADHD/ADD medications, etc…) including over-the-counter (OTC) medications (such as Tylenol, Ibuprofen, etc…)  (Health provider only needs to complete page 1 of this form)

  1. Asthma Action Plan for School: Use this form if your child has asthma and/or needs Albuterol at school. This form works as a medication permission form and a health care plan for students with asthma.

  1. Allergy/Anaphylaxis (Severe Allergy) Emergency Care Plan: Use this form if your child has a severe allergy and has an epi-pen prescribed. This form works as a medication permission form and a health care plan for students with allergy/anaphylaxis (severe allergy).

 FAQ 3: What if my child has other health/medical needs not covered by the 3 forms above? (such as diabetes, seizures, g-tube feeds, oxygen, etc…)

For any other health/medical needs at school, please reach out to your school nurse consultant and/or to your school health assistant and we will provide guidance on what orders/plans will be required to provide the needed health services at school. 

 

 FAQ 4: What if my child needs a primary care provider or other health care services (Behavioral health, Dental Care, Pharmacy, Maternity care, Adult Primary Care, etc…)?

Sheridan School District partners with Sheridan Health Services, Inc. which has two clinics serving adults and children. The Sheridan Family Health Clinic and the Sheridan Youth Health Clinic both provide primary care and a comprehensive range of physical, behavioral, oral health, and pharmacy services for both children and adults. Both clinics are conveniently located in Sheridan close to the schools.
